Missing pallet-rack anchors: Inspection and escalation guide

Do not diagnose from an empty hole

Base plates can contain holes that are not all used in every approved configuration, while an apparently present anchor can still be loose, damaged, incorrectly located or unsupported by the required documentation. Observation must be compared with the specific rack, slab and design information.

Do not use a visual guide or calculator to approve continued service. Protect people from the affected area, prevent additional loading or impact, record the condition, and obtain the decision required by the rack manufacturer, engineer, employer procedure and applicable authority. Do not probe, torque, remove or test an anchor during a routine visual walk unless that task is part of an approved procedure performed by qualified personnel with the area controlled.

Use the approved anchor and slab information

Collect rack drawings, anchor schedule, manufacturer information, installation records and known slab details. Anchor type, diameter, embedment, edge distance, spacing, hole condition, concrete properties and rack loads can matter to the design.

A replacement that fits the hole is not automatically compatible. Drilling near an existing hole or into unknown embedded services creates additional concerns that must be addressed in the approved corrective scope.

Verify work and update the register

The closeout should identify the approved anchor, installer, location, drilling and cleaning method where applicable, field exceptions, required inspection and return-to-service authorization. Preserve photographs and supporting documents.

Review whether rack impact, guard contact, slab deterioration, installation variance or undocumented configuration change contributed. Include adjacent bases in the review when the evidence supports it.

Record anchor conditions without disturbing them

The observation packet should locate the condition precisely and preserve the evidence needed for system-specific review.

Base-plate map

Record which plate holes appear occupied, the visible anchor condition and the orientation of the plate. Use a consistent diagram or photograph annotation without declaring a required count.
